There are two pharmaceutical agents commonly used to treat MS fatigue; modafinil and amantadine. These two drugs offer significant benefit for some people, but also have side affects. There is also great interest in the use of alternatives to these agents to relieve fatigue, one of which is Acetyl l Carnitine.
Acetyl-l Carnitine (or ALCAR) is an amino acid that helps transport fat into the body’s cells. Once inside the cell, the mitochondria oxidize the fat to produce the energy that drives the activities within our bodies.
Evidence suggests that people with MS may be carnitine deficient. L-carnitine is made inside our bodies and is taken in through food, mostly meat and dairy products. Supplementing the diet with ALCAR may be an efficient way to increase a person’s energy level by increasing the amount of fat (or fuel) in the cell.
Research in this area is limited, but at the University of Rome, scientists compared the effect of amantadine vs ALCAR on fatigue in 36 people with MS. The study was small, but the results, which were published in The Journal of Neurologic Sciences, indicate that ALCAR is very effective in battling MS fatigue and has fewer side effects than amantadine.
Because safety has not yet been established, the National Multiple Sclerosis Society does not recommend its use. The study noted above used the dose of 1 gm per day, but the ideal, safe dose has not been determined. Also, people on certain medications should not take this supplement, particularly those with low thyroid levels. Always check with your physician before taking any supplements.
